Forge sensor: pipeline_alert_slack_broken retry + escalate

feature_id: FEAT-FORGE-RELIABILITY Detect: AlertHistory row with slackTs like failed-% within 24h. Heal (autonomy=auto): retry postSlack (2x backoff). If both fail escalate via alt channel then mark escalated-alt-<ts>. Suppress duplicate Post-incident Ledger tickets.
